CalOptima’s Program of All-Inclusive Care for the Elderly (PACE) is a community-based Medicare and Medi-Cal program that provides coordinated and integrated health care services to frail elders to help them continue living independently in the community. PACE provides all the acute and long-term care services covered by Medicare and Medi-Cal. The services are arranged for each individual PACE participant based on his or her needs as assessed by the PACE Interdisciplinary Team. A key element of the program is attendance at the PACE center, where participants get medical and social services, including visits with a primary care doctor, physical and other therapies, recreation, social activities and meals. To qualify for PACE, a person must be 55 years or older, live in the PACE service area and meet the nursing facility level of care
